The patient is on a dose of Neurontin 100 mg tid, which is much too high and is causing her to be confused. Therefore, I have decreased the dose to 200 mg bid.
A possible defense-pt was concerned they were taking the medicine too often so it was changed to less often. I have done all kinds of crazy appearing things in order to get patients to take meds they need. Or maybe the doc is math challenged.
Haha, I am sure the pharmacists loved that order.
