Not a diss. I would think that when everything is fresh and new you're more likely to dot all your Is and cross your Ts. As you move on you start to take shortcuts.
That Attending speaks the truth, the level of detail and energy that a med student puts into writing an H&P is much different than what an Intern or Resident has the time to do.
