Typos

A typo from a sign-out that made me crack up:

"If patient is agitated, OK to give small dose of fativan."

  1. Transcription error on an x-ray report: Instead of "A patchy area of infiltrate in the LLL ..." the report was transcribed as: "An Apache arrow infiltrates the LLL ..." Here is my question - How did they identify it as an Apache arrow?
    This one I actually saw in a med rec: I saw Mrs. Smith-Jones at 0600. She looked sicker than snot. Arranged for transfer to ICU.

    Patient: I have low blood pressure

    Dr; Ohhhhhh postural hypotension!
