Transcription error on an x-ray report: Instead of "A patchy area of infiltrate in the LLL ..." the report was transcribed as: "An Apache arrow infiltrates the LLL ..." Here is my question - How did they identify it as an Apache arrow? This one I actually saw in a med rec: I saw Mrs. Smith-Jones at 0600. She looked sicker than snot. Arranged for transfer to ICU.
