Dictation error

I know our dictation system relies on an automatic dictation program, which is  then edited by a human being. But sometimes I see an error that does not seem like it could have been made by either the dictation program OR a human being:

"Patient will be seen for blood pressure management technique that would management blood pressure management."

How did that error get made?


  1. The computer had too much alcohol to drink?

    2. Probably the same way the providers I transcribe for advance the colonoscope to the second and third portion of the duodenum, prescribe levothyroxine in milligrams instead of micrograms, and interchange he/she in their reports--a split second lapse of concentration.
