I was in sports clinic yesterday afternoon so I picked up the chart for the next patient and started reading the progress note from the previous visit to see what we were seeing him for.
Mr. [Patient] is a 39 year old gentleman who sustained several anterior dislocations of his right shoulder when he was involved in martial arts as part of an underground fighting group. He did not seek treatment for these and the problem worsened after he lifted heavy weights while in prison. He presents today with his caretaker, who states that because of his impulsive and self-destructive behavior secondary to bipolar disorder with psychosis, it is difficult for him to follow any sort of treatment plan.
We definitely attract the classy sort over here.
That isn't funny, it is sad. And it's mean to make fun of someone who obviously has issues, even if they are a difficult patient.
ReplyDeleteI did not read that as funny or mean nor do I see any indication or claim that he was a difficult patient. Why is it sad? WHO are you to judge. So what if he enjoys fight club, do you know why he was in prison? Just because he does not let the laws of other men chain him up does not make him a bad person. You are the mean one trying to act like you are better than everyone else. Dr. Orthochick was just sharing, why you want to be so mean and try to keep her from sharing? Well Big Ole Miss Judgemental I got a question for you, now that the internet is here, do you burn phone and cable lines instead of books?
DeleteNo, more like holding him up for ridicule for being in prison and being an underground cage fighter. Maybe he became a criminal because he's a bad person. Maybe he's a criminal because of untreated bipolar disorder with psychosis. And are all cage fighters automatically vulgar? That's the point, we don't know. So why join in with judging and smirking?

DeleteI am sorry if my comment came off as judgmental. What bothered me was the sarcastic description of the patient as "upscale" and "classy sort". It seemed like he was being made fun of for his history of incarceration, fighting group participation, and bipolar w/ psychosis. I do not think that the patient is a bad person, nor did I imply that.

Remind me of my notes at the beginning of medical school. I would put the patient story the way he said it (as we have been told, but apparently academic medicine is way different than practical). I kept wondering why my doctors smirk every time I present. Oh good time.
ReplyDeleteLighten up a little, folks. This is not your average history and you know it. I don't think Dr. Orthochick is trying to ridicule this patient. Taken in the context of her other posts, with that background, I sincerely doubt it. She has shown great subtlety and sensitivity in her posts in general… It's just a lot more colorful than 'tennis elbow.' And, frankly, I think it's okay to say that underground fighting and prison are not 'classy.' I don't think that Dr. Orthochick is saying that being 'classy' is the loftiest goal. She is pretty much saying - look at this unusual history I read today.
